Current valid vehicle registration or
Current year's W-2 form(s) or
State or Federal Tax Return filed withing the past 12 months with W-2 form(s) attached.
Business returns do not meet Residency requirements.
3. ONE of the following ORIGINAL DOCUMENTS with parent/guardian's name and address:
Property Tax Bill with parent's name and property address indicating homeowner's exemption
or
Lease/Rental Agreement with parent's name, student's name and address, as well as manager or owner's name and telepone number.
Immunization Records
Yellow Immunization Card or similar document
Mantoux TB test results completed within last six (6) months for students entering
from outside Santa Clara County
Proof of Varicella (chicken
pox) immunization for students entering from outside
the State of California. Same form as Mantoux TB test.
Transcript from previous school with check-out grades
Current IEP or 504 Plan if your student is receiving
special services
Emergency Card completed and returned (orange card) -
Pick up the card from the Guidance Office.
Once all the above documentation is completed, please
call the Los Gatos High School Registrar at (408) 354-2730
x 231 to schedule the enrollment appointment for your
student. Documents may be faxed to Los Gatos High School
at (408) 354-3742.
