| Before you may enroll your student, you must
provide all of the following information to the Registrar
at Los Gatos High School:
Proof of Residency (see above residency packet):
1. Parent's Picture ID:
- Current California State Driver's License, or
- Current California ID Card or
- Valid Passport or
- Consulate Issued Picture ID.
2. ONE of the following DOCUMENTS with parent/guardian's name and address:
- Current valid vehicle registration or
- Current year's W-2 form(s) or
- State or Federal Tax Return filed withing the past 12 months with W-2 form(s) attached. Business returns do not meet Residency requirements.
3. ONE of the following DOCUMENTS with parent/guardian's name and address:
- Property Tax Bill with parent's name and property address indicating homeowner's exemption or
- Lease/Rental Agreement with parent's name, student's name and address, as well as manager or owner's name and telepone number.
Immunization Records
Transcript from previous school with check-out grades
Current IEP or 504 Plan if your student is receiving
special services
Emergency Card completed and returned (orange card) -
Pick up the card from the Guidance Office.
Once all the above documentation is completed, please
call the Los Gatos High School Registrar at (408) 354-2730
x 231 to schedule the enrollment appointment for your
student. Documents may be faxed to Los Gatos High School
at (408) 354-3742.
